• NSAID or Non Steroidal Anti-Inflammatory Drug (EG Motrin, Aleve, Meloxicam) Use Before Rotator Cuff Surgery Increases Failures

  Although the general public is generally not aware of it, it is well established in the medical literature that NSAID drugs interfere with healing. For this reason in particular I never use them when I perform reparative surgery where they might compromise the surgical repair. This has been well shown in various animal studies but there have few studies in humans where it has been looked at. However in a new study from Norway the use of NSAIDs before surgery was correlated with a greater chance of failure afterward. This is quite significant in that the drug use did not have to be during the period of healing to impair healing later. The most common drugs in this class are Ibuprofen (motrin, advil), Naproxen (Aleve), and Aspirin over the counter and Diclofenac, Meloxicam and Celebrex by prescription.

  Total knee and hip replacement are wonderful restorative procedures - except when they aren’t. While it is well known that pulmonary embolism, heart attack, stroke and death occur at surgery in a small percentage of patients, it is less well known that infection of a total joint replacement can equally be, or more, devastating. Infections occur in about 2% of patients. At a recent symposium of the American Academy of Orthopaedic Surgeons, new research showed that THE FIVE YEAR MORTALITY RATE OF PATIENTS WITH AN INFECTED TOTAL JOINT REPLACEMENT IS HIGHER THAN FOUR OF THE FIVE MOST COMMON CANCERS. Repeat surgery and intensive antibiotic therapy is required in nearly all cases, but resistant antibiotics prevent eradication of the infection in some. And the physical and emotional stress of the treatment over a period of years takes a heavy toll on the patient.

  A few weeks ago I saw a 90 year old retired doctor, whose physician daughter brought him in, with one completely ruptured Achilles tendon and one partially ruptured. Usually these require significant trauma – which he did not have. What was the cause?
